Instruction to clients:
  1. Make a letter requesting for a Resource Speaker to lecture / discuss on a particular health concern. The letter should be very specific when it comes to the HEALTH TOPIC to be discussed, TIME and DAY, the PLACE to where the lecture is to be held. The letter should be addressed to the CITY HEALTH OFFICER, DR. FLORENCE G. REYES with the address: HEALTH SERVICES OFFICE, T. ALONZO ST., BAGUIO CITY.
  2. It is recommended (but not required) that the letter should likewise indicate if the host could provide the equipment (like Sound System, LCD Projector, Computer / Laptop, Over Head Projector) for more effective delivery of the lecture. This information helps us decide on what lecture materials we'll be bringing on the lecture site.
  3. There are cases that the lecture request is in conflict with other lecture schedules or activities of the office, so it is strongly recommended that the request letter SHOULD BE AT LEAST received at the office one week before the lecture schedule.   1. Go to the cashier and pay the amount of the test required. For pre and post natal patients, hemoglobin, hematocrit and blood typing are free of charge. For under-five children, hemoglobin and hematocrit are free. Physical Examinations of government employees like hemoglobin determination is free. Government employees are also entitled to a 20% discount on other hematology examinations.

  1. What is Rectal Swab and its purpose?
    - Rectal Swab is a method used in the collection of specimen from the rectum or anus. Its purpose is to diagnose patients with Typhoid Fever which is a contagious food and water-borne disease caused by Salmonella typhosa
  2. Who are subject to Rectal Swab?
    - The following are to be swabbed:
    1. all employees of all food establishments;
    2. all employees of bars, hotel and restaurants
    3. food vendors;
    4. students of Hotel and Restaurant Management (HRM) course who are on their job training (OJT)
  3. Where can we avail of Rectal Swab?
    - Rectal Swab can be availed of at the Public Health Laboratory located at the Basement of the Health Services Office Building; every Mondays, Tuesdays and Wednesdays; limited to 100 patients per day on the afore-mentioned days.
  4. How much is Rectal Swab?
    - Rectal Swab is done free of charge to all who are getting Health Certificates.
  5. How long do we have to wait for the result?
    - A maximum of seven (7) days or a minimum of five (5) days is needed in the processing of one (1) specimen.
  6. Do I have to wait for the result of Rectal Swab before I can process my Health Certificate?
    - No, you can already process your Health Certificate even without the result of your Rectal Swab.
  7. What if I am positive for Rectal Swab?
    - If a patient turn out to be positive for Salmonella/ Shigelle, he/she will be treated; another Rectal Swab will be done after to determine the efficacy of treatment.
  8. Can we have Rectal Swab done in other Laboratories?
    - No, only the Public Health Laboratory of HSO in authorized to perform such test for Health Certification purposes.

Long Form for Health Certificate (HC) of Entertainers
(SOCIAL HYGIENE CLINIC [SHC])
Description: 1/2 lengthwise long bond paper

   
 
Instruction to Clients:
  1. Upon initial visit at Social Health Certificate (SHC), register at the counter by listing under the daily smear logbook.
  2. A long form will be issued for completion of requirements at the Social Health Certificate and Public Health Laboratory (PHL)
    1. Updated negative smear
      - Signed by the SHC staff
    2. Updated negative chest x-ray (renewed every 2 years)
      - Signed by x-ray staff
    3. Negative stool result
      - Signed by PHL staff
    4. Syphilis & HIV testing
      - Pay (P250.00) at the cashier (if not a surveillance period) for the tests to include Health Certificate fees.
      - Go to SHC and secure Syphilis and HIV testing No. to be signed by any SHC staff
      - Go to Serology, PHL, for blood extraction
    5. Plus:
      - Police Clearance from BCPO
      - Working permit Form from the City Hall Treasures Office
      - 3 pcs 2 X 2 ID picture
      2 for the working permit
      1 for the greencard
      - 1 pc 1 X 1 ID picture for the pink card after completion of long form
      - Greencard to be left at the SHC (filled-up at the SHC)
  3. The SHC physician signs long form and greencard and countersigns working permit.
  4. Working permit signed by City Health Officer, City Mayor's Office, Treasurer's Office (P150.00) and BCPO ( 1 working permit taken).
  5. After the working permit has been completely signed the entertainer goes back to SHC. For completion of Pink card:
    - filled up by the SHC staff
    - countersigned by the SHC physician, to include latest smears
    - signed by the City Health Officer
  6. The pink card should be carried by the entertainer every time they come to the SHC. For smear and to make sure that the results are written down by the SHC staff.
  7. The pink card should always be brought to the right establishments and shall be collected by the floor manager every time the entertainer reports for work.

Sexually Transmitted Diseases / Serology
(PUBLIC HEALTH LABORATORY - SEXUALLY TRANSMITTED DISEASES / SEROLOGY SECTION)

Description:

  • Laboratory request form that is properly and completely filled up. Request form must be signed by the requesting physician.
  • Commercial Sex Workers should get long forms first with their entry number at the Social Hygiene Clinic. Long Forms should also be properly and completely filled up.
   
 
Instruction to Clients:
  1. Go to the cashier and pay the amount of the test required. Syphilis testing for anti-partum patients are free, government employees are entitled to a 20% discount on serological examinations. Commercial sex workers should secure long forms at the Social Hygiene Clinic and get their entry number.
  2. After paying, proceed to the laboratory, present request form, or long form together with the receipts.
  3. Collection time is 8:00 - 11:00 AM and 1:30 - 4:00 PM from Monday to Friday.
