NOTICE TO THE PUBLIC

July 8, 2022

 

Subject: Application for Accreditation of Civil Society Organizations for Representation in the Local Special Bodies

The Sangguniang Panlungsod ng Baguio is inviting all CSOs that are operating within the City of Baguio to apply for accreditation. Attendance to the CSO Conference on August 5, 2022 at 8:30 AM at Hotel Veniz Session Road is required before applying for accreditation.

For inquiries and other details, kindly look for Arch. Donna Tabangin, City Planning and Development Officer, at the City Planning and Development Office or call (074) 442-8905); or Dr. Jonalyn Pagada, Head of the Sanggunian's Agenda and Information Management Division or call (074) 444-2343. Both offices are located at the right wing, ground floor of Baguio City Hall

Please submit one (1) copy each of the following to apply for Accreditation of your Organization to the Sangguniang Panlungsod ng Baguio:

  1. Letter of Application (download template here)
  2. Duly accomplished Application Form for Accreditation (download template here)
  3. Duly approved Board Resolution signifying intention for accreditation for the purpose of representation in a local special body (download template here)
  4. Certificate of Registration or existing valid Certificate of Accreditation from any NGA (or in the case of IPOs, certification issued by NCIP)
  5. List of current Officers (download template here)
  6. For CSOs that have been in operation for at least one (1) year, Minutes of the Annual Meetings of the immediately preceding year as certified by the organization’s board secretary or Certification from the board secretary certifying the annual meeting’s conduct, including the date, location, attendees, and agenda (download template here)
  7. For CSOs that have been in operation for at least one (1) year, Annual Accomplishment Report for the immediately preceding year (download template here)
  8. For CSOs that have been in operation for at least one (1) year, Financial Statement, at the minimum, signed by the executive officers of the organization, also of the immediately preceding year, and indicating therein other information such as revenue, expenses and the source(s) of funds (download template here
