FRONTLINE SERVICE: Issuance of Social Case Study Report/ Referral Letter
Requirements:
For Medical Assistance:
- Medical Certificate/ Medical Abstract
- Updated Hospital Bill or latest doctor’s prescription with amount indicated and stamped by hospital or pharmacy
- Barangay Certificate of Residency
- Valid identification Card
Fees/ Charges: None
Duration: Seven (7) Working Days
How to Avail of the Service:
- Registration of client at the logbook
- Social Worker conducts an interview to the applicant to gather relevant information and will advise the applicant on what are the requirements to be submitted
- Applicant will submit the requirements to be reviewed by the Social Worker and assess the eligibility of assistance.
- Applicant will be given a schedule of home visit for further data gathering and validation
- Social Worker will conduct home visit, make collateral interviews and issue claim slip to the applicant
- Preparation of Social Case Study Report/ Referral Letter by the Social Worker
- Release of SCSR/ Referral Letter to the Applicant

- Proceed to City Health Services Office (CHSO) at T. Alonzo St., Baguio City
- Fill up the information Sheet and applicants will be advised to attend the required seminar on scheduled date.
- Attend the Pre-Marriage Counseling (PMC) Seminar on scheduled date
- Issuance of PMC Certificate to applicants for submission to the Local Civil Registry Office
FRONTLINE SERVICE: Food Assistance

Duration: One (1) Hour
How to Avail of Service:
- Registration of the Client at the Logbook
- Social Worker conducts interview and assessment to the applicant
- If qualified, social worker prepares Requisition Issuance Slip (RIS)/Requisition Slip and instruct the Administrative Support Staff for the issuance offood items to be given to the applicant
- Provision of food commodities to the applicant

3. Proof of Age: Birth certificate, Passport or any valid ID indicating one’s birth date

HOW TO AVAIL THE SERVICES:
- Registration at the logbook
- Social Worker conducts an interview and initial assessment to the applicant to gather relevant information and will orient the applicant on the requirements to be submitted.
- Applicant will be given a schedule of home visit for validation and further data gathering.
- Social Worker shall conduct home visitation and collateral interviews.
- Applicant will submit the requirements to be reviewed by the social worker.
- Social Worker will assist the aplicant in the preparation of Feasibility Study/ Project Proposal.
- Social Worker will give the Contract to the applicant for signature and notarization.
- Applicant will submit the notarized Contract to the social worker.
- Preparation of voucher by the Social Worker.
- Social Worker submits the documents for approval and processing of assistance
FRONTLINE SERVICE: ISSUANCE OF SOLO PARENT IDENTIFICATION CARD
Requirements:
- Barangay Residency Certificate
- Affidavit of two disinterested persons (for unwed, abandoned, separated, widow/widower for more than a year)
- Income Tax Return/pay slip (for working applicants)
- Photocopy of Birth Certificates for minor children (bring original Birth Certificates)
- Medical Certificate for applicants whose souse are disabled (original)
- Certificate of Legal Separation (original)
- Certificate of Finality of Marriage (original)
- Certificate from Jail for applicants whose spouse is in jail for one year
- Death Certificate (bring original)
